Moderator's Biography
Mr. Samuel Chang is the Deputy Director of Hospital Quality Management Department of Changhua Christian Hospital and the Taiwan Patient Safety Culture Club Surveyor. He is a qualified Patient Safety Officer certified by the Institute of Healthcare Improvement (IHI), the Clinical Audit certificated by the Healthcare Quality Quest, and received the Human Factors Analysis and Classification System Program certificate. Mr. Samuel has been devoting himself to medical quality and patient safety works for almost 20 years. That includes healthcare quality index monitoring, using root cause analysis for accident investigations, preventing an emergency by using failure mode and effects analysis, and team resource management.
Mr.
Samuel also dedicated to academic researches and published results in domestic
and international conferences such as the International Society for Quality in
Health Care (ISQua), Asian Society for Quality in Health Care (ASQua), National
Patient Safety Foundation (NPSF), International Hospital Federation (IHF),
International Forum on Quality and Safety in Healthcare (IFQSH), Patient Safety
Movement Foundation (PSMF), Taiwan College of Healthcare Executive (TCHE),
Joint Commission of Taiwan (JCT), and Taiwan Healthcare Quality Association (THQA).
He is also committed to training medical personnel overseas to continue
providing the knowledge and promoting healthcare quality and safety, such as
Papua New Guinea, St. Vincent, St. Lucia, and other Southeast Asian countries.
